Adding help text to a form for multiple single select check boxes

I am trying to convert a form from Smartsheet but can’t seem to get AirTable to do it. I have multiple single select check boxes. Right now I can add help text but only to each individual check box. I want to be able to have a ‘header’ explaining that they can make as many selections as they need to. I don’t want the data to be multiple selections in the same cell. This would require me manually making 1000+ selections for the existing data I am trying to port over.
Alternatively, is there a way to just have a floating text box that you can add to any segment of a form.
Bonus question: is there anyway to have check boxes stacked with the check box aligned to the right rather than underneath the title/question of each check box?
