Adding in a list to a table

I am trying to create a form for room booking, I want a field where the organiser fills in the name of the attendee complete with a separate column for contact number. I don’t want to get this information from another data base as each meeting will be with unique attendees, so a linked data base would not work. In addition if all 600 users have to add the name of the attendees with telephone number in a data base that is then looked up it creates GDPR issues. The object of the attendees section is to have a contact number for track and trace should another attendee fall ill after the meeting. So all I am looking for is a 2 column word type table where the attendees name sits along side attendees contact number.

Does anyone know if this is possible?
