Record answers to a standardized test

Hello everyone,

I'm new to Airtable.

I need to administer a standardized test consisting of several questions to several people every month.
I would like to be able to record the answers so that I can follow the evolution of the answers over time and according to the people.
I can create the list of questions and record the answers, but I can't create separate entities for each date on which the questionnaire is taken.

Could you help?

Hmm I don't think I'm really following what you're trying to do.  I've put something together here and if you could let me know what you need done differently I can see what I can do to help!

It's not really what I want.
In order to make my wish more explicit, I have just made a diagram:

Hmm, so you need each question to be a single record?  If so, how does this look?

The workflow for getting people to fill this in would need some figuring out though.  Having it easy for people to fill in would make the data unstructured, and so we'd need to use automations to format the data afterwards

Getting people to fill in the data in a structured way is possible but would be really tedious for them to do.  Could you provide an example of the form you're asking them to fill out to see what we're working with?
