There is also a setting in Facebook to automatically post to instagram when you post to Facebook. But you may not be doing the same things with Instagram that you are doing with Facebook, so I'd be carefull with that one.
If you use fillout.com instead of jotform, they support it. They also support a TON of other cool stuff, like displaying information on the form based on options chosen, etc..
