How to autofill a field in a form in one base based on a field in another base

Assuming I have the following form in one base:

And the following table in another base:

table

  1. I want the entry in the zip to correspond to a table in another base meaning the zip entered should be in that table

  2. The rate field in the form should autofill based on the date entered. For example, if a user makes an entry in the month of Feb, this “rate” field should autofill from “meal_feb” corresponding to the zip code the user has entered.

I am new to Airtable and I could really use some help.

If you want to do this entirely within Airtable, you would need to sync the table with the information you want prefilling the form into the base hosting the form. Then how you prefill the form would depend on where people are accessing it from. How are people accessing the form right now?
