airtable interface ... when adding a new dependency field how do I edit the form that comes up

Not sure I even know how to ask this question... hopefully screen shots will help..

I have two tables Clients and Contacts. 

I'm trying to figure out how to edit the new contact screen that comes up when I add a new contact from a Client record as below:

1. When I add a new client I want to then add a new contact so I click on the new contact button below:

2. And I get the usual list. but the contact is new so I click the new contact button at the bottom:

3. And get this screen below which doesn't have half the fields I need.

How do I edit this screen?
