Form Submission Error (Duplicate Entry)

Question if anyone received the following error with a form submission. This error has not occurred with this form for a few weeks until the last few days. The data is still processed though into AirTable however, because of this error the end user doesn’t know if it is processing correctly or not.

Only changes that were made since in the last week was AirTable API connection through FiveTran to BigQuery.
