As far as I am aware there is no way to configure Airtable’s “forms” to add your own HTML elements
A better idea would be create your own form using HTML(so that you can add HTML elements) and then using the Airtable API save those values to your base on Airtable

Andrew’s suggestion is, of course, a good option. However, if you don’t want to code you could simply embed your simple Airtable form into a page containing your ‘verbiage’. Obviously this would mean that your input boxes would need to be together which may or may not work for you.
Another approach would be to use a different webform app (like Cognito Forms for example) which would allow you to build a more custom form in the first place - you could then transfer the data to Airtable with a simple Zap (in Zapier).
