Unfortunately, that is not natively possible with Airtable's forms. Airtable's forms will only show you the primary field.
You could change your primary field to be a formula field that includes everything that you want to see, but that can get pretty unwieldy.
Otherwise, your best bet is to use Fillout which is FREE, and it is the most advanced form software available for Airtable today. It does exactly what you want, and sooo much more.
