Utilizing Forms to have people update their information

My end goal is to create a form (linked to an airtable of course) where someone could input their:

First Name
Last Name
E-Mail
Phone
Title
LinkedIn
Company

  • Categories (of what type of industry they are in)

  • URL of Company

  • City, State, Country

Conferences Attended

  • Panels spoken on

The categories are only linked to a company. I want to figure out how I can set this up so that when people are inputting into a form they can see what other categories have been defined.

Right now I am having trouble having the category that is connected to a company showing up in the form.
