Entering data while restricting view

I have 5 supervisors that I am wanting them to "review" workers for each shift of an event.  Ideally, I want a way for the supervisors to be shown a blank table where they write their own notes for 6 different fields for each worker without seeing the notes that other supervisors have written.

I don't care if they see their old notes...or if their entries disappear from their view altogether after filling in the data.

I would need to be able to see the entire table with everyone's entry.

Is this a possibility?

The only way I can think of is through interfaces. You should create a separate interface for each supervisor, and within each interface, you can expose the data required for the supervisors through views. And on their interfaces they also can edit and write notes. It's important that you grant supervisors access only to the interface relevant to them and not to the base.

If they don't need to see their old entries a form seems like it would work too
