Hello! 

I am looking for an advanced questionnaire, either in Forms for Airtable or an integration. I would need it to be in sections (each section having a little header with directions), and an if-then conditional type of response, where if they answer a question one way, the next thing pops up based on that answer.

Any thoughts on how to do this in Airtable or what integrations may be best for this?

Airtable's Interface forms seem like they'd do the trick.  You could have your sections with titles and headers:

And you'd be able to set your fields to display conditionally based on the answers to previous questions:
