Looking for an Advanced Questionnaire

I am looking for an advanced questionnaire, either in Forms for Airtable or an integration. I would need it to be in sections (each section having a little header with directions), and an if-then conditional type of response, where if they answer a question one way, the next thing pops up based on that answer.

Any thoughts on how to do this in Airtable or what integrations may be best for this?

You’ll want to use Fillout’s advanced forms for Airtable for that.

Fillout is 100% free and offers hundreds of features that Airtable’s native forms don’t offer, including the ability to conditionally hide static text elements or fields based on the logic that you define, the ability to create multi-page forms with conditional paths, the ability to update Airtable records from a form, the ability to display Airtable lookup fields & Airtable rollup fields & Airtable attachments & formulas on forms, dynamically & conditionally filter linked record fields by any values that you would like, perform math or other live calculations on your forms, accept payments on forms, create new linked records on a form, display as many fields as you want to see in a linked record selection list (including attachments), connect a single form to dozens of external apps simultaneously, limit the number of linked records that can be chosen, upload an unlimited amount of attachments simultaneously, add CAPTCHAs to your form, add choice matrixes to your forms, direct integration with hundreds of apps like Calendly & Google Maps on your forms, and so much more.

Airtable's Interface forms seem like they'd do the trick.  You could have your sections with titles and headers:

And you'd be able to set your fields to display conditionally based on the answers to previous questions:
