Previously there is information on the related linked record similar to what we see in the base. But now the information is no longer there.

Unfortunately, Airtable doesn’t show that information on forms, for some strange reason.

To get that information to show up on forms, you would need to use an external tool that supports that feature.

Fillout is the only tool that I know of that supports this feature. I’m not sure what other tools support this feature.

Yes, for some reason other fields are displayed when you view the form in the form builder in Airtable, but when the form is deployed via an external link, only the primary field is displayed. It may have been this you were seeing before.
