When click 'open' in an interface, two forms are shown

When click 'open' in an interface, why two forms are shown?

Sometimes only 1 form is shown (which is what I expect), but often two same forms are shown.

Anything I did wrong?

Interesting!  I just tried to replicate this but wasn't able to, and hopefully someone else has an idea.

If you could share an example copy of your base without any data in it I'd love to poke around to see what I can figure out!  In the meantime I'd suggest opening a support ticket too

There's definitely a bug in the forms in the interface at the moment, we have an interface which presents a form for the add record button, but it is not the form that is available to edit when editing the interface, nor is it the form that we have specified to be used. There's some kind of default form that isn't being exposed to the editor.
