Form View configuration question for Speaker Evaluations

I have a table with Presentations and another table for Speaker Evaluations.  The Presentation Title is a Linked Field in the Speaker Evaluation Table to link the two tables.  I also have a Look-up Field for the Speaker's Name.  I am making a form for the Speaker Evaluations.  The first item is to choose the presentation.  I would like the look-up field for the speaker to show up in the form as non-editable so the evaluator is reminded of who they are evaluating.  The form currently will not show it.  Is there a workaround?

Try using on2air forms, I think they can do this. I think there is a free signup at the moment. https://on2air.com/forms/
