Welcome to the community, @Meagan_Beltekoglu! :grinning_face_with_big_eyes: Jotform can definitely help with what you need, but it can’t do it on its own. Jotform’s native Airtable integration only works to add new records, and not all field types are supported. Link fields, for example, can’t be selected via the built-in integration. Because you’ll likely want the parent to choose their child’s name from a list based on another table, it’ll require some outside help. You could put together your own configuration using tools like Zapier or Integromat, but a fully-functional tool like On2Air: Forms (by @openside) would be a better option.
