Jotform’s native Airtable integration only works to add new records, and not all field types are supported. Link fields, for example, can’t be selected via the built-in integration. Because you’ll likely want the parent to choose their child’s name from a list based on another table, it’ll require some outside help. You could put together your own configuration using tools like Zapier or Integromat, but a fully-functional tool like On2Air: Forms (by @openside) would be a better option.
