New fields are displaying in grid view but not in the form

I am in the middle of creating a form and have had no issues, but all of a sudden the fields I am creating are displaying in grid view but not in the form.

That's...odd.  For what it's worth I just tested creating new fields in a Grid view and they showed up for selection in the Form view fine:
