Is there a way to automate a Slack or Email message based on criteria in multiple cells?

I work with several clients and want to send automated messages to our team if it has been more than 5 days since the last point of contact.

Ideally the body of the email would contain the name of the client and the date of the last point of contact, as follows:

• Client A 21-07-2021


• Client B 20-07-2021


• Client C 19-07-2021
