Send form with fields attacted

I work with a cohort of independent trainers who offer in-person and only training. I have a column with my training courses listed. At the end of the training, I would like to send an evaluation form to my participants that displays the course and the trainers as listed in my airtable base. Is this possible with a url with fields attached?
