Booking system for a team of teachers

Hello!
I am designing a booking system for a team of teachers. You can use the form to book a meeting (video call) with one of the teachers.

My problem is that for a single teacher, I can do it, but for a group of teachers I can’t.

If anyone can help me, I would be very grateful!
