Auto Filling Adobe Document

Hello I have forms I have to fill out for different state governments. These forms are all fillable PDF documents which I have attached into my base. Is there a way to use the information entered in the system to find the correct document (based on state from another table) and then autopopulate the PDF document?

Appreciate the help!

Visualize a single spreadsheet. Uploaded PDF form templates are listed on the left. Saved (filled in) forms are listed on the right.

Use "Duplicate" option to auto-fill data from one filled in form to another instance of the form. Send live shareable link to any form, so recipient can fill in the rest of the form and submit. The completed form is submitted back to the original "spreadsheet". Very simple. Those who fill in shared forms do not need to install any special software. it just works.
