I am migrating a base and form from SmartSheet - on that form, there is a check box for “Send me a copy of my response” and when checked, it reveals a box to input your email address. Is this possible in Airtable?

That option is only built into Airtable’s old form views, which you can get to from the “Data” layer under “Views”, which is where you can create a new form view.

However, note that this option only works for users who are signed into Airtable with an Airtable account, so it wouldn’t work for the general public who might be filling out your form.

If you’re using Airtable’s new forms, this option isn’t built into those forms, but you can create your own automation to email the person after the form is submitted, and you can include whatever information you want from the form to appear in the subject & body of your email.

Alternatively, if you want to create a form that already has this capability built directly into the form, you can always use Fillout’s advanced forms for Airtable, which is 100% free and just lets you toggle this option on or off.
