Automate Creation of Report/Invoice Type - Document

I am willing to pay for the project if someone is able to show me how/set up the scenario relating to the following...

I am trying to create a report from one of my records. The record has multiple fields with multiple linked records I want to display in one document. 

It's important the the document is also downloaded in word, so edits can be made if needed. I have make.com, zapier accounts.

Page designer is able to accomplish the basic design I am looking for ( I have shown a picture below), but I am now wanting to automate this with a checkbox. 

I understand this can be done in make.com however I do not have experience with JSON (which I believe you need).

I am willing to pay for the project is someone is able to show me how/set up the scenario
