Uneditable prefilled form fields

I don't think that's possible with Airtable forms at this time I'm afraid.  I'd suggest trying Fillout for this instead: https://www.fillout.com/

An additional benefit would be that Fillout would allow the users to edit the records directly instead of creating new records too

Hi Isobel,
(1. if you are using Airtable Team plan)
You can set the fields you don't want users to edit as "Read-only" in the field settings. 
So what happens is that though users will be able to see these fields as normal, they won't be able to actually edit them.

(2. If you are using airtable free plan)

Use a linked record in the form, and allow the record to show all relevant fields. (Users can see but not edit each field shown)

(3. Finally), Add a checkbox field in your base and airtable form (for each field that needs confirmation) where users can tick "Yes" or leave it unchecked for "No". (PS: You can use single select field as well if you prefer that).
