Getting an error while setting up an extension

Trying to use this to create records based on a template and everything looks correct, but when we ran, it returns an error.

I'm assisting another user with their Airtable project and just looking for possible solutions.

User downloaded the script from Airtable Marketplace.

Here is the tutorial we followed. We used the same fields as a test and received the error.

I believe its the scripting extension
