Assuming you want to keep your form structure of having all the questions in a one page form, you're going to need to use an automation that triggers every time a record gets created in "Form" to create records in the appropriate tables

You might want to consider creating links between your tables instead though; that way when users are submitting the form in Fillout they can create the records in Clients, Services etc themselves
