I have created a form view for each employee (5) that is personalized to them, with their name and a fun image for them at the top. It is used to track their billable time per client. In the grid view, however, I’d like to see which form (or person) submitted each line so that I know who completed the work. Is this possible?

+35You could have them choose their name from a single select field on the form when they fill out the form.
You could even prefill each form with their name in that field by giving each one of them a customized link to the form that has the prefill value in it:
