Hi All I am creating a New Patient Form which requires the patients physical address set out as following. Address line 1 Address line 2 Address line 3 City Country Postcode Is there a way of creating one Question on the Form (ie Address) that bring up those fields as answer boxes to that question? I'm afraid not. You could try a hacky workaround where you add the three fields and make the fields for Line 2 and Line 3 look empty, but the space will be all weird:
