Since it works for you but not the rest of your team, I would look at what is different about them rather than the form itself.

Is the form set to “Only people with access to the base” or requires sign-in?  If so, check their permission level and whether they’re logged into the right account.  As a temporary test try making the form public to see if that helps, then change it back once you know whether it’s the cause.

Another check is to see if there are any conditional visibility rules or linked record fields.  If a field was renamed, deleted or had a select option removed, the form can break for some users.

Finally you can have them try the form in an incognito window that has no extensions.  If nothing there helps send Airtable support a screenshot and the exact time it happened.  A server error is worth reporting.
