List of records base on user input

Hi,

I’m looking for a way to replicate one Google sheet I’m using. It’s use to populate a table base on the user input. The user must enter our PO number or the client PO number, then a query fetch the data.

User can use this table to send a email.

Does anyone have a suggestion ?
