Need to add buttons to a form and have it reflect a table on the other side

so, we've created a general intake form

I added to this form

"BECOME A TENANT UNION MEMBER!
Become a member, receive additional benefits
Select an option"
but this does not reflect on the table connected to the form.
 
furthermore, I want to add buttons to take the person to payment websites. Can someone offer to call me or walk me through what I can do. its not so much integration, its more like added buttons and making sure the data reflects on the table.

    Could you provide screenshots of your form and your table and indicate what information you'd like reflected on the table?

    You can't add buttons that do that to Airtable forms I'm afraid.  You could maybe put links into the description sections of the fields and ask users to click on those, but it'd be kind of confusing I think

    A tool like Fillout.com may be your best bet for something like this. The airtable forms are somewhat limited.
