Pivot Table woes: missing months and different column widths

We have a pivot table to review a variety of reporting, but currently have 2 major issues:

1. Months with no data are not being represented, making it look wholistic when in fact its not. IS there a setting im missing or a way to force empty months into the reporting?
2. Column headers are getting squished making headers difficult to read. Is there a way to force same width for all columns? Or at very least to also take into account column headers as minimum width (not just the values)?
